Initial In-Home Care Setup Visit
A personalized home walk-through to identify opportunities for safety, comfort, accessibility, and caregiver efficiency. Together, we'll discuss equipment needs, daily routines, mobility, transfers, bathing, repositioning, and practical caregiving considerations. We'll also review your Relief Point Guide, journal, and log book so you feel prepared and supported from day one.
Closing In-Home Support Visit
A final in-home review focused on reassessing needs, identifying adjustments, addressing ongoing concerns, and developing a sustainable plan for continued success after the program concludes.
